Intake Transporter Jobs in Rancho Cucamonga at Hannah’s Children’s Homes
Must have non expired passport or CA Real ID
Intake Transporter will provide transportation to the minor’s resource parent’s homes after the intake is completed and assist with intake duties. Transporters will also be available for fly outs to transport children to sponsors in the U.S.
Responsibilities and Duties:
- Reviewing and arranging all transportation paperwork inclusive of driver logs fligh information, trip reports and invoices
- Routine cleaning and servicing of the passenger vans
- Establishes and maintains communication with clients and coworkers to improve quality of service and customer satisfaction.
- Transport clients inside and outside of the facility to designated areas in a punctual and timely manner and assists with reunification transports and flyouts
- Routinely complies with the standards set forth by CARF and HIPAA.
- Regularly participates in ongoing education to improve knowledge and skills.
- Assisting with Creating UC Files
- Creating discharge Packets
- Preparing admission packets
- Preparing admission welcome bags
- Entering UC cases into online database Extended Reach
- Entering Resource parent cases into online database extended reach
- Entering admission and discharge case actions into extended reach
- Assist with filing for all departments
- Assisting Clinical Dept. prepare materials for group therapy.
- Assisting Clinical Dept. prepare materials for Youth-Led Community Meetings
- Assisting Education Dept. prepare materials for after school Community Group.
- Assisting Education Dept. in preparing Ed materials for the next school day.
- Assisting Medical Team complete first aid kit checklist and restocking.
- Assisting Medical Team restock medical supplies such as Lice Kits, gloves, hair bonnets.
- Assisting CM Team upload to Portal UC and Sponsor documents.
Transporter Skills and Qualifications
- High School Diploma or GED (Preferred)
- One year in experience in transporting clients
- Current CPR certification or must be obtained within 30 days of hire
- Excellent communication skills
- Impeccable organization and time management skills
Job Type: Full-time
